How to apply for the Thomas B. Fernald Scholarship

Requirements:  Former Student of Shady Grove Elementary
Criteria:  High school senior, active in “community service” – unpaid, volunteer efforts aimed at helping a population in need, a well-rounded individual, an overall good student, and a person of good character. 
How do I apply?  Provide the following by April 11, 2018
Resume that includes:
  • Name, Address, Phone Number
  • College or University planning to attend or have applied to
  • Years Attended Shady Grove Elementary
  • Grade Point Average
  • Summary of Community Service and TOTAL Hours (see attached example of format).Please provide a brief description of the organization and your role.
  • List of Activities and employment (see attached format below)
  • A letter of recommendation from someone who is familiar with your community service and character.
  • A Letter from your Guidance Counselor verifying the years you attended Shady Grove Elementary
  • A one page maximum reply to the writing prompt:
Essay
Shady Grove Elementary School PTA is a parent run volunteer organization in a public school in Henrico, Virginia. Parents and teachers volunteer their time and talent in many ways to enhance the education of the children. The Thomas B. Fernald Scholarship honors students who have used their time, talent, or education to be a leader in their community.

Please share with the review committee what led you to the opportunity of community service and how the experience has impacted you as well as those you have served.  Feel free to expand on leadership and/or discovery of talents throughout your experiences.
